Download the latest Phothera fax form and complete all information, digitally or handwritten, and simply fax this form, including the patient’s insurance card and chart notes to 216.765.0271 to submit the prescription.

The Phothera physician written order form covers the 100XL, 200, 600-3D, and all other home devices. For the fastest and most accurate workflow, it is recommended to complete the prescription through an EMR, like EMA or Epic, or our e-form.

Important notice: The ClearLink Portal has been retired as of September 2026. Please use another option to prescribe at-home phototherapy for your patients.


Important safety note: a patient may experience mild, transient erythema during the course of treatment. Erythema can result in as little as 15 seconds of exposure, or approximately 200 millijoules of dose. Patients should stop treatment until the reddening resolves and communicate with their prescriber.

  • It is recommended for patients to use lip balm and/or sunscreen with SPF ≥ 30 applied to non-treated areas.
  • Do not use over skin eruptions
  • The patient and anyone in view of the device must wear the provided UV blocking glasses or goggles designed to block 100% of all UVA and UVB light from the eye area when worn
  • Do not remove protective eyewear, or any other protective equipment, during treatment
  • All other people and pets should leave the area to avoid unwanted exposure to ultraviolet light
  • If psoralens (photosensitizing treatment) are being used as part of the treatment, the eyes should be protected from exposure to ultraviolet (sunlight) for 24 hours after taking the treatment
  • Patients should center their body between the lamps during treatment to avoid over exposure to isolated areas of the body
